Assumption of risk document (PDF) - Complete this form when requesting use of the tracks or gymnasium in any building within the district. Each person using the facility in the group needs to sign they voluntarily assume any risk involved while using the facility. The signed copies must be submitted with the building use request.
Building use request (PDF) - Complete this form when requesting use of any facility within the three buildings in our district. The form must be returned to the building you are requesting use for.
Cafeteria deposit form (PDF) - This form should be submitted to your child's school when making a payment on your child's breakfast/lunch account.
Emergency release & notification (PDF) - This form should be completed each year or when there are changes to information with contact information for parents/guardians, additional release/contacts, and medical information/power of attorney. One should be submitted to each building you have a child in.
Enrollment Form (PDF) - Complete this form and return to the school your child will be attending. For more information and guidelines on registering your child in school visit the new student enrollment website.
Medication request authorization (PDF) - School nurses cannot administer medication to students during the day without written consent from the physician and parent or guardian. This form needs to be completed and returned to your child's school building.
Social History Form (PDF) - This form is completed for a family history on new elementary students entering the district of for students being referred to the Committee on Special Education or school counseling.
